Compassionate Care Creates A Path To Safety And Stability
History: Member is a 61 year old Caucasian male who was admitted to nursing and rehab facility due to homelessness and unmanaged behavioral health. Member is originally from Greece and migrated here with his sister over 10 years ago.
Barriers: Member was not being seen by any behavioral health provider in the community and was not on any psychotropic medications. Member was homeless and has no access to natural support. Member met and married a resident that resided at Lake Front Terrace. Member then allowed his wife to move into his apartment once transitioned. Member was a victim of domestic violence on December 5, 2025. Member was physically assaulted and was sent to the hospital for treatment. Member’s medications were also destroyed during this incident after being flushed down the toilet. Member was treated and sent home the same day. Member was discharged by the hospital to “ The Livingroom”. Which is a space for discharged patients to stay for a few hours during cold weather.
Due to the aggressor remaining in the members apartment, the CTI Team arranged for the member to be admitted to the Holiday Inn in Evanston, IL. Member was also provided food and water. CTI CC Pleasant ensured member was safe by visiting and communicating with the member by telephone several times a day. CC ensured member was discharged with prescriptions to ensure his medications would be replaced. CC waited for the member’s prescriptions to be refilled and delivered them to the hotel. CC assisted member with reading the prescription bottles to ensure he understood dosage instructions. Member’s wife had access to all of the members identification including State ID, Social Security Card, Green Card and Debit Card. CTI Team assisted member with cancelling the debit card to ensure the member will have access to his SSI funds in January 2026.
Interventions: Member enrolled in the Community Transition Program in July 2025. Member chose to transition independently to the community with the support of the CTI Program. Members sister resides in Greece and is in daily contact with the member by phone. Member has established a PCP in the community as well as a Psychiatrist. Member was also assessed by Catholic Charities for Aging waiver services. Member was assessed by Thresholds for outpatient therapy services and CST services.
Member reported that he was not comfortable returning to his home due to his wife continuing to reside there. An Abuse and Neglect report was filed with APS. Member was then transferred to Safe Haven shelter that will allow him to remain in the shelter until alternative housing is located. Member has toured Grand Regency of Jackson Park for Supportive Living Placement.
Outcome: Member has been residing at Safe Haven shelter since the incident happened. Since the member has been in the shelter, the CTI team has shown a tremendous amount of support in ensuring the member is as comfortable and safe as possible. CC have been in close contact with the member on a daily basis. CC continues to call and check in on his health, needs multiple times a day, and CC is seeing member face to face several times a week to ensure safety, support, and presence are felt. CCs are in contact with his immediate family to also ensure that he is safe and out of harm’s way. The CTI team is currently working diligently and expeditiously to find the member a new safe and comfortable place to call home. CC has made sure the member is able to receive the services that he needs, specifically being able to find the member a new primary care provider. The CTI team has shown a great sense of care, commitment, and curiosity to ensure the member is getting the help and support that he deserves during this time.
