Meeting Members Where They Are To Create Paths Of Stability And Support

John is a member of Blue Cross Community Health Plans. For a long time, life was very hard for him. He struggled with constant paranoia and often went to the emergency room. He also had trouble with the justice system, including short jail stays for things like petty theft and missing court dates. During this time, John used alcohol to cope and did not want help from professionals.


At first, it was difficult for the Care Management Team to connect with John. He did not respond much or engage in services. However, his mother played an important role. She stayed in touch with the team and helped keep communication open. Even though progress was slow, this steady support helped build a small amount of trust over time.


After many months, something began to change. On Feb. 10, 2026, John made an important decision and signed medical and legal documentation to begin the process to improve his life. This was a big step, and it showed that he was starting to trust others and was more open to getting help.


From that point on, John continued to make progress. He had a full conversation with the Care Management Team, which was something he had not done before. He also became open to working with Trilogy and started exploring housing options through the Blue Cross and Blue Shield of Illinois and Trilogy partnership.


Today, John is in a much better place. He is actively taking part in services and is more willing to accept support. His journey has not been easy, but over the past eight months, he has shown real growth. John is now on a path toward greater stability, safety and improved well-being.
