CountyCare Helps Wheelchair-Bound Teenage Member And His Family

Charlie* is a 13-year-old CountyCare member who sustained a severe spinal cord injury resulting in the permanent loss of motor function in his lower extremities. He is dependent on a wheelchair for mobility. Charlie’s recovery was complicated by multiple hospitalizations for serious conditions including sepsis, dehydration and heart issues.


Due to the complexity of his needs, Charlie’s parents reached out to his CountyCare care coordinator and together they reviewed his interdisciplinary plan of care (IPOC). Through regular outreach and ongoing communication with his parents, his care coordinator helped to ensure his appointments were scheduled and completed with his primary care provider, specialists and rehabilitation services.


Charlie’s care coordinator learned about his love for the water and coordinated for him to attend aquatic therapy a couple of times a week. Charlie’s care coordinator continues to provide his family with the guidance and support needed to manage Charlie’s complex medical conditions so they can collectively work toward his best possible quality of life. Charlie was able to return to school full-time and spend time with his friends.
