IAMHP Pathways Survey
HFS, in collaboration with our Medicaid MCOs, is gathering information from providers related to the Pathways to Success services that are being implemented throughout the state.
Pathways to Success provides enhanced services and supports for Medicaid enrolled children under the age of 21 in Illinois who have complex behavioral health needs and require intensive home and community-based services. Eligible youth have access to two tiers of care coordination including:
Tier 1 – High Fidelity Wraparound for children and youth with the most complex behavioral health needs.
Tier 2 – Intensive Care Coordination for children and youth with complex behavioral health needs.
Pathways to Success also covers additional services that can be provided in the youth's home and community including: Intensive Home-Based Services, Family Peer Support, Therapeutic Mentoring and Respite.
The state of Illinois and Medicaid MCOs are committed to the success of the youth served under Pathways to Success and would like your feedback to continue to strengthen the provider networks and overall healthcare experience for these children.
We are interested in receiving feedback from providers to determine the best way to move forward with continuing to develop the provider network needed to serve these especially vulnerable children.
